Job Description:
Case management positions consist of advocating and assisting adult clients experiencing chronic substance misuse and/or severe mental illness and involves direct, written, and telephonic contacts with clients and their service providers to improve outcomes and patient satisfaction and to ensure access to care and needed supports.
DESCRIPTION OF DUTIES:
Assist clients with accessing recovery and community resources/supports Assess client's strengths and needs:
medical, housing, education, vocation and socialization Serves as the contact point, advocate and informational resource for clients, treatment team, family/caregivers, and community resources Facilitates access to appropriate treatment providers through inquiry, referral, coordination and timely follow-up Increases continuity of care through managing relationships and communicating information and/or facilitating delivery of documentation to all providers especially during transitions in care Utilizes person centered approaches including Motivational Interviewing when interacting with clients Build and maintain positive professional relationships with community partners, clients, and co-workers, adhering to the code of ethics and agency policy Monitor client's progress and complete documentation within LifeSkills timeframes Have a knowledge of diagnosis and treatment of health, mental health and substance use disorders Complete continuing education requirements Perform other duties as assigned by Supervisor.
Qualifications:
BA degree preferably in human services related field including psychology, social work, or sociology or be a Kentucky Licensed Practical Nurse.
One year of experience working with adults experiencing severe mental illness or chronic substance.
Must have valid KY Driver's license and automobile insurance coverage.
SKILLS AND KNOWLEDGE REQUIRED:
The ability to work with persons in a manner that shows respect for cultural differences is needed.
Must possess organizational, oral, and written communication skills.
Computer skills are required including a high level of familiarity with Microsoft word.
All 30
hour positions will be enrolled in our company retirement program through the Kentucky Public Pension Authority (KPPA); six percent (6%) of your gross income will be deducted from each check.

employees who work 30 hours or more a week are required to participate in this program.
The employee contribution rate is set by the State General Assembly.
Employee contributions are deposited into member accounts.
These contributions to the retirement system are currently tax deferred.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
